Applicant Statement of Commitment & Photo Release

TLI requires participants to attend all scheduled weekly sessions and one full-day Saturday leadership retreat, as well as complete all assigned tasks. Attendance at monthly recollections is highly recommended. Only individuals willing to make this commitment should apply.

I understand that the tuition cost for the program is $2,000. If admitted into the program, I am responsible for submitting the fee prior to the orientation, which is the first session of the program.

I understand the mission, goals and requirements of TLI, and if selected, will complete the program in its entirety. After completion of the program, I intend to assume a more active leadership role as a lay Catholic in my community and professional career.

I hereby give the program administrators right and permission to use, re-use, publish and republish photographs, articles, video and audio recordings taken of me and/or my family members during TLI, for use in publications, websites, social media, and/or advertising. I waive the right to inspect or approve finished photographs, video, written copies, public relations copies of printed matter that may be used or created and appear in connection therewith. I understand that said photographs will remain under the control of the program administrators. I hereby release the program administrators, its representatives, employees, and assigns, from any claims, liabilities and damages arising out of the rights granted hereunder or the exercise of. I declare that I am at least 18 years of age.
